How to Get a Level 1 Fingerprint Clearance Card for DDD Caregiving

Safety is the top priority for the State of Arizona when it comes to self-directed care. Before you can be hired to provide Attendant Care, Habilitation, or Respite—even if you are providing that care to your own family member—you must pass a strict background check.

The most critical piece of this background check is obtaining a Level 1 Fingerprint Clearance Card issued by the Arizona Department of Public Safety (DPS).

What is a Level 1 Clearance Card?

Unlike a standard pre-employment background check, a Level 1 Fingerprint Clearance Card involves a continuous, state-and-federal criminal history check. It specifically scans for offenses that would disqualify someone from working with vulnerable populations, such as children and adults with developmental disabilities.

If your record is clear, DPS issues a physical card that remains valid for six years.

The Step-by-Step Application Process

Navigating the DPS portal can be confusing. Here is the standard process for family caregivers:

  1. Start the Application Online: You must visit the Arizona DPS Fingerprint Clearance Card portal to set up an account and begin an application.

  2. Select the Correct Reason: During the application, you will be asked why you need the card. For DDD caregiving, you will typically select the option related to "DES - DDD / Developmental Disabilities."

  3. Pay the Fee: There is an application fee (usually around $75). Keep your receipt, as many AWC agencies will reimburse this cost after you have worked a certain number of hours.

  4. Get Fingerprinted: Once your application is submitted and paid, you will receive a reference number. You must take this number to a contracted fingerprinting location (like a Fieldprint office, which has multiple locations across the Phoenix metro area) to have your prints digitally scanned.

Do Not Let Paperwork Stall Your Paycheck

A common mistake family members make is waiting until the last minute to apply. DPS processing times can vary wildly from a few days to several weeks.

At Nova Z Health Services, we make the fingerprinting process a day-one priority. Our administrative team walks new AWC caregivers step-by-step through the DPS portal so you don't select the wrong category or delay your own hiring process.
